Report all injuries to your supervisor or school nurse at the time of injury. An Employee’s Report of Injury Report MUST be completed within the first 24 hours of injury. Once completed and signed by your supervisor or principal please send to Jessica Vooght in Employee Benefits. If you are leaving your duties to seek medical attention please notify your doctor and/or hospital emergency room that the Pearl River School District is self-insured with:

Please use your social security number until you receive a workers compensation case number from Wright Risk.
If you are unable to report to work due to an on-the-job injury, please call your absence in as such and give the date of injury. You must also let Jessica Vooght (845) 620-3937 know this as well so she can notify the Workers Comp Board and complete the proper paperwork. When you return to work you must have a medical note stating you can return without restrictions.
Out-of-State Residents who seek a doctor regarding work-related injuries are asked to consult a New York based doctor, unless the injury is of an emergency nature. Injuries occurring within the School district come under the jurisdiction of the New York Workers’ Compensation Board. As such, the Law requires that you be treated by a doctor authorized to treat Workers’ Compensation cases who will charge what it permissible under the New York State fee schedule. If you need assistance in choosing such doctor in your vicinity, please call (718) 802-6600. Any unpaid doctor or hospital bill that you may receive should be forwarded to Wright Risk Management at the above address.
Employees who are out of work due to on the job injury cannot return to work and are to remain off district property unless they have a medical note (with a diagnosis) stating that you can return to work full duty,NO RESTRICTIONS and have clearance to do so from the district. Paychecks will be mailed to your residence. -

Need to set up an 403b, 457b, ROTH 403b Account? OMNI403B
A listing of our participating OMNI providers can be found on the OMNI website. You will set up a meeting with whomever you choose. They will set up your account and contact OMNI on your behalf. OMNI will then notify the district that you have set up a deduction amount with a start date. Each account is personal to you. It is up to the individual to work with their provider for election amounts. Changes are not made through the business office. You will deal directly with OMNI or your financial officer.
You can use the OMNI website to make changes to your elections through an SRA (salary reduction agreement)
Click on Employees and then Start/ Change Contributions.
Under Salary Reduction Agreement Forms for: Select New York. Employer Name is Pearl River CSD. You will be redirected to the SRA Agreement Page. At this point you can complete your changes online.

To make changes to your payroll tax deductions please log into Wincap ESS.
- Once logged in hover over Employee Self-Serve on the purple ribbon until the yellow pop up opens. (Do Not Click)
- On the yellow popOnce up you will see "My Employee Deductions" Click on this
- The top box will show your current allocations. Below this box you will see a purple Add Change Request. Click here Complete the new form and select Update. Please consult your financial advisor if you have questions about your selections.
Changes to your W-4 or NYS IT-2104 are not an immediate update. Your request will be reviewed by payroll and may take up to two pay cycles for the changes to be completed.
